Do you feel that you could benefit from speaking to a health counselor today?
If not today, what day and time would you be available?
If you are unable to decide on a day and time, one will be generated for you automatically according
to your geographic location.
You will be obligated to the appointment once it has been generated.
Submitting this form now will include you into our system.
You must be in the system to be assigned a unique "identification number"
Without an identification number, you will not have access to food,
water, shelter, clothing, etc;
You must be in the system to survive.
Not being in the system will have an unknown and/or negative impact on your well-being.
Your identification number will be yours until membership expiration or repossession...
Which ever comes first...
