I'm invisible
Despite all the people who walk by
Busy with their own lives
No second glances
All I can think about is you
And how a small misunderstanding
Turned into a million bad thoughts
But nothing worse than you hating me
Completely alone
Sitting in the dark
Wishing you knew the pain and hurt
That you are causing me

The one who hurts the most is always the one left behind. The other one is already onto bigger things. Sadly for you. Best get into another relationship quickly. Move on.

perhaps people have an idea of the hurt which is why they inflict it - or do they just lash out?

The poem provides the insight to the reader of your emotions and pain.
Maybe this poem provides you a therapeutic outlet, which is good. My limited advice - don't isolate. Talk things out. If you were in the wrong, apologize. If they were expect one. My impression is you are a young lady, attractive and pretty bright. Everyone wants to be loved, but don't let in enslave you. I think you'll know what's right. If it doesn't feel right, move along.
